CONSUMER HEALTH DATA WE COLLECT

MHMD provides certain rights to Washington Consumers with respect to their Consumer Health Data, subject to certain exceptions. You have the right to access and request deletion of your Consumer Health Data we may have collected, shared, or sold. If you provided consent for collection or sharing of your consumer health data, you have the right to withdraw your consent. In addition, you have the right to appeal our refusal to act on your request. If your appeal is denied, you may submit a complaint with the Washington State Attorney General at www.atg.wa.gov/file-complaint.
